Community Health Access and Navigation in Tennessee (CHANT)
CHANT is an integrated model of care coordination provided through the local health departments. The program provides enhanced patient-centered engagement, assistance with navigating complex systems, and care coordination of medical and social service needs.
Families and individuals are connected to available resources which may include reimbursement for medical services and assistance with co-pays, deductibles, and co-insurance for children and youth with physical disabilities.
Goals for the CHANT program include increasing Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) rates, medical and dental home access, pregnancy, birth, maternal and child outcomes, and positively impact overall outcomes related to social determinants of health.
Who is eligible?
Individuals eligible for CHANT include:
- Pregnant and postpartum adolescents and women
- Children (Birth - 21 years)
- Children and youth with special healthcare needs (Birth - 21 years)
If you’re a provider with a family that may benefit from the CHANT care coordination program, use this form for referrals. Self-referrals are also welcome.
Women, Infants, and Children (WIC)
The WIC program provides supplemental foods, nutrition education, and breastfeeding support to pregnant and postpartum women, and children up to age 5.
The role of the WIC program is to educate healthcare professionals and the public about pediatric nutrition and provide feedback regarding available formulas, feeding practices and tolerance.
Therapeutic Formula Requests
Qualifying conditions for the requested prescribed formula must be consistent with indications recommended by the manufacturer. Approval will be contingent on this and Tennessee WIC policy. If you have questions regarding a request, please contact the WIC nutrition program manager at 865-215-5052.

WIC supplemental foods that are offered to participants are aligned with the Dietary Guidelines for Americans and the American Academy of Pediatrics infant feeding guidance.
Tobacco Cessation Resources
KCHD offers a Knox County Tobacco Prevention Training for employers, which will benefit anyone who serves people using commercial tobacco, from healthcare professionals to social workers and school staff.
The Tennessee Non-Smokers Protection Act of 2007 allows Tennesseans to breathe smoke-free at numerous restaurants, hotels and other indoor establishments.

Power to Quit
Power to Quit is a FREE program that offers private counseling in a judgment-free space to help pregnant women and their families quit smoking. Partners/spouses can also join and receive incentives like free diapers and wipes.
